An Accountable Care Organization (ACO) is a group of doctors and other health care providers who work together. The goal of an ACO is to keep people healthy by offering affordable, high quality, patient-centered care.

How will I benefit from patient-centered care and working with doctors in an ACO?

How do you coordinate my care?

Our doctors, nurses, and other care providers share your treatment and health goals with each other. This care team has secure access to your medical history (including past medical conditions, prescriptions, and visits to the doctor). This helps us work better together. It makes sure that everyone has the most up-to-date information and your best outcomes in mind. We will work with you and your health care providers to deliver quality care, when you need it.

I'm concerned about sharing health information, can you tell me more?

To provide quality care, we want your doctors, nurses, and other professionals to know your most up-to-date health information. At Live Oak Care your care team views your health history to coordinate your care. Sharing information keeps your care team current with your medical needs and treatment. It helps us provide you with quality care. It allows us to help keep you healthy. And it's more convenient for you. You don't have to provide each care provider with the same information. You'll be able to avoid repeated tests because your doctors have your medical information.

Please know that your privacy is very important to us. The privacy and security of your medical information is also protected by law. We share information to assist with the delivery of quality care.
